Community Living and Home Supports

Community Homes

Provides a home for two to six people with 24-hour support for older individuals living with developmental/intellectual disabilities. Each home enhances the individual's interests and abilities, and an emphasis is placed on community involvement, stability, and maintaining important ties to families and loved ones.

Life Sharing through Family Living

Life Sharing through Family Living assists individuals with developmental/intellectual disabilities who desire to live with the support of a loving and caring family or companion in the community.

The Calvin and Morris Hassel Residence

This 18-bedroom residence serves seniors with intellectual disabilities who do not require skilled nursing care. This Personal Care Home provides a peaceful environment in a beautiful tree-lined neighborhood.

Community Residential Rehabilitation Services

Provides community homes for three to six individuals with mental illness. With a focus on rehabilitation, the staff provides individuals with the support and training needed to participate in and prepare for everyday life in the community.

Supports for Independence

Home Assessment and Modification

Assists eligible low-income, aging adults so that they can remain safe and secure in their own homes. Simple changes such as installing shower grab bars and removing tripping hazards can help eliminate the risk of injury and provide a safer and more secure environment.
Additional languages spoken: Cambodian, French, Italian, Laotian, Russian, Spanish, Thai, and Ukrainian. Translation services for other languages available.
