Supports for Independence

In-Home Supports: Attendant Care

In-Home Supports: Attendant Care empowers individuals with physical disabilities to enjoy the comforts of home by providing them with personal care. The program enables individuals to stay in their own homes, work, attend school, participate more fully in their communities, and maintain more comfortable, independent lives.

Services

  • Personal care attendants scheduled at the consumer’s requested days and times
  • Assistance with activities of daily living, including bathing, dressing, and grooming
  • Meal preparation and feeding assistance
  • Light housekeeping, laundry, and errands

Eligibility

Must be age 18 to 59, mentally alert, have a physical disability that will last at least one year, and reside in Philadelphia, Bucks, Montgomery, Delaware, Chester, Lehigh, Berks, or Northampton County. Services are also available to residents of all other counties in Pennsylvania.

Cost

Services are funded by the PA Department of Public Welfare, and there are no costs for financially eligible individuals on medical assistance or Supplemental Security Income (SSI). Fees are assessed on a sliding scale basis for individuals who do not qualify for free services.

In-Home Supports: Fiscal and Payment Agent Services

In-Home Supports: Fiscal and Payment Agent Services provides personal assistance and administrative support to assist seniors, adults with physical disabilities, adults diagnosed with mental retardation, and their families. In this model of care, the consumer is the employer of the person providing their direct support.

Fiscal Agent Services

  • Bathing, dressing, grooming, toileting
  • Processing payroll
  • Arranging and managing workers’ compensation
  • Record keeping
  • Employer training and ongoing support
  • Criminal/child abuse records check
  • Technical assistance/employer supports
  • Assistance with hiring, training, firing, and managing the in-home workforce

Payment Agent Services

  • Payment to vendors and providers of approved services
  • Assistance with personal budget management, administrative reports, technical assistance, and consumer training
  • Assistance with purchasing necessary services including personal assistants and respite care, habilitative services, and home and vehicle modifications

Eligibility

  • Must be an authorized, service eligible resident of the funding county
  • Services are available in counties throughout Pennsylvania; call for more information

In-Home Supports: Personal Assistance Services (PAS)

Care comes easy when its provided by someone you trust.  In-Home Supports: Personal Assistance Services (PAS) empowers seniors to hire the caregiver of their choice, including a friend or relative, who will then assist with bathing, dressing, grooming, meal preparation, mobility, and light housekeeping.

Services

  • Bathing, dressing, grooming, toileting
  • Performing criminal background checks
  • Arranging Worker’s Compensation
  • Issuing paychecks to personal care assistants including tax withholding
  • Maintaining local, state, and federal tax records for you and your assistant
  • Conducting orientation for seniors and personal care assistants
  • Assisting with recruiting, hiring, firing, and training of in-home employees

Eligibility

  • Must be age 60 or older
  • Services are offered in counties throughout Pennsylvania

Home Assessment and Modification

Through simple construction and modifications, such as installing shower grab bars and removing tripping hazards, Home Assessment and Modification allows aging, eligible adults to remain safe, secure, and comfortable in their own homes.

Services

  • Assessment of home environment by licensed occupational therapist
  • Recommendations for low and no-cost home adaptations to increase safety and independence
  • Small grants available for home modifications

Eligibility

  • Participants must be low-income seniors who reside in the Philadelphia area
  • Other eligibility criteria may apply; call for more information

Nursing Home Transition

Through Nursing Home Transition, we help fulfill the desires of individuals looking to transition out of a nursing home and return to living on their own in the community. Services include one-on-one support from a qualified coordinator, help with locating housing, planning the move and setting up the household, assistance with security deposits, adaptive equipment, utilities, and assistance with learning budget management.

Services

  • One-on-one support from a qualified coordinator
  • Assistance in finding housing, planning your move, and setting up your household
  • Financial help with expenses, including security deposits, household items, adaptive equipment, utilities, and assistance with learning budget management
  • Facilitate returning to and/or modifying an existing residence
  • Referrals to home- and community-based services such as transportation, Personal Assistance Services, peer support, assistive technology, and home modifications

Eligibility

  • Must be age 18 years or older, qualify financially, reside in a nursing home in Pennsylvania, and be interested in returning to the community
  • If a person qualifies to receive Nursing Home Transition services, they are eligible for up to $4,000 in goods and services to set up their new home

Nursing Home Transition is a collaborative effort of the PA Department of Aging, DPW Office of Long Term Living; Governor’s Office of Healthcare Reform; local County Assistance Offices; local Area Agencies on Aging; local Centers for Independent Living; home and community-based providers; and organizations representing nursing homes.
