JEVS Supports Coordination assists individuals who have a physical disability to help them stay in their own homes, go to work, attend school, and participate more fully in the community. Our services center around independent living, consumer control in choices, and inclusion.
- Supports coordination
- A dedicated Service Coordinator will help program participants create a personalized care plan and provide ongoing support, which may include reevaluating care plans and hours of care
- Assistance with arranging for home modifications, durable medical equipment, assistive technology, transportation, discharge planning, personal emergency response systems
- Prevention of nursing home admission
- In-home visits are scheduled at dates/times of the program participant’s choice
Program participants must be age 18 or older and have a physical disability that will last at least one year. Services are available to residents in all counties in Pennsylvania. Contact Supports Coordination for further eligibility details.
Additional Languages Spoken
Russian, Spanish, Armenian, Cambodian, American Sign Language. Translations for other languages available.
Services may be free for eligible individuals; for others a co-pay may be assessed. Services are funded by the Pennsylvania Deparment of Human Services Office of Long-Term Living.
